


FIVE EASY ADMISSION STEPS
1. Complete the Family/Student form along with a one-time non-refundable $100 application fee.
2. Fill out the Authorization to Release Records and submit the following:

Birth Certificate

Baptismal Certificate (if applicable)

Medical Insurance Card
-
Original of the student's State of Florida School Entry Health Exam (DH3040)

Original of the student's Florida Certification of Immunization (DH680)

*Please note the Diocese of Venice does not accept Religious exemptions for immunizations.

3. Schedule a time for your student to take an entry assessment.
4. Complete and sign the Financial Commitment Form.

Once the Admissions Committee has reviewed your application and your child has been accepted, pay the subsidized $450 registration fee, the $100 iPad fee (for grades 6-8), and the $100 graduation fee (grade 8).
5. Begin the scholarship application process, if needed.

Financial aid requests are made by completing the FACTS Grant and Aid Assessment application online at FACTS. A FACTS Grant and Aid Assessment is required for consideration for any scholarships, tuition assistance, parish subsidy, etc.

Setup a tuition payment plan using the FACTS Tuition Management System at FACTS.

A nominal fee, administered by FACTS, applies when you enroll in a tuition payment plan.
